Application Requirements
- Application of CCNL industrial managers
Documentation required
- Legible photocopy of tax code or health card
- Enrolment form completed in its entirety
- Declaration, stamped by the company or on its company letterhead and signed by the legal representative of the company, containing the text of the “Company Declaration”
- Consent to the processing of personal data
The quarterly premium will be paid by the company, deducting the amount payable by the member from his/her relevant earnings.

RETIRED MANAGERS
Retired manager
Retired industrial manager who has attained the right to retire (contributory retirement pension, old age pension, invalidity pension) and who has accrued the required years of membership.
Requirements
- Accrued period of Fasi membership:
- managers enrolled when working who have been Fasi members for at least 10 years, provided u>that no more than 8 years have elapsed between the date of termination of employment and the date of retirement;
- managers who have not been Fasi members for 10 years, provided that they have worked for at least 10 years in companies that were already enrolled in the Fund on 01.01.19 and that are members of a substitute Fund, provided – in this case too – that no more than 8 years have elapsed between the date of termination of employment and the date of retirement.
- Application for enrolment as a retired manager submitted within six months of retirement date (for the purposes of eligibility for enrolment and calculation of the necessary conditions, deferment of retirement due to obligatory legal provisions will not be considered).
Required documentation
- Copy of certification proving retirement, or copy of application for pension showing that the right to a pension has been attained
- Statement from previous company/companies certifying the date of commencement and termination of last employment as an industrial manager (in the event of employment in more than one company, a declaration must be submitted for each)
- Completed registration form
- Consent to the processing of personal data

RETIRED CONVENTIONALLY ENROLLED MANAGERS
Retired manager
An industrial manager who has attained the right to retire (contributory retirement pension, old age pension, invalidity pension), who has not attained a length of Fasi membership of at least 10 years, will be able to enrol conventionally (i.e. by paying a premium equal to that paid by the manager and the company).
Requirements
- Accrued period of Fasi membership::
- Retired managers who have not accrued 10 years’ membership of the Fund as per the previous section “Enrolment of retired managers”, but who have worked for a period of less than 10 years at companies that were already members of the Fund on 01.01.2019 and that use substitute funds to assist their working managers, provided that no more than 8 years have elapsed since the date of termination of employment.
- Application for enrolment as a retired manager submitted within six months of retirement date (for the purposes of eligibility for enrolment and calculation of the necessary conditions, deferment of retirement due to obligatory legal provisions will not be considered).
Documentation required
- Copy of the certification proving retirement or copy of the pension application showing that the right to a pension has been accrued
- Declaration by the former company/companies stating the date of commencement and termination of last employment as an industrial manager (in the event of employment at more than one company, a declaration must be submitted for each)

The relevant quarterly premium, equivalent to the sum of the premium payable by the manager and that payable by the company, will be paid by the retired manager him/herself.
HOLDERS OF WIDOW’S/WIDOWER’S OR SURVIVOR’S PENSIONS
Holders of widow’s/widower’s or survivor’s pensions of working managers at companies paying the increased Article G premium, and therefore not members of the Fund at the date of death, can apply for enrolment.
Documentation required
- Copy of the certification proving retirement or copy of the pension application showing that the right to a pension has been accrued
- Declaration by the former company/companies stating the date of the commencement and termination of the deceased person’s last employment as an industrial manager (in the event of employment at more than one company, a declaration must be submitted for each)
- Completed registration form
- Copy of identity document and health card
- Consent to the processing of personal data

The relevant quarterly premium will be paid by the holder of the widow’s/widower’s or survivor’s pension him/herself.
Holders of widows’/widowers’ or survivors’ pensions of managers registered with the Fund at the date of death may, on the other hand, request the membership to be transferred as described in “How to maintain membership”
WORKING MANAGERS WITH CCNL DIFFERENT TO THE CCNL FOR INDUSTRIAL MANAGERS
Employees of companies having a CCNL (national collective labour agreement) other than that signed by the Associates, but nevertheless signed by at least one of them, or by a national organisation relating to one of them, may enrol with Fasi provided that the said CCNL allows enrolment in the Fund itself by working managers.
Moreover, employees of Confindustria-associated companies to which a CCNL other than that signed by the Associates applies may also enrol.

ENROLMENT OF FAMILY MEMBERS
The manager (or Fasi member), may apply for the extension of cover to his or her family members pursuant to Article A of the Regulations.
To take effect simultaneously, any extension of cover to family members must be applied for when the manager enrols, indicating the family composition on the application form.
Family members (spouse/civil partner, cohabiting partner and children) may also apply for enrolment at a later date than that of the member’s own enrolment: this will take effect on the first day of the month following the application.
Requests for the enrolment of parents can only be made at a later date if they become dependent on the member after the member’s enrolment; here too, membership will begin on the first day of the month following the application.
The family members covered may only submit refund claims through the main member. The refund will be sent only to this person as the sole holder of the right to services.

ENROLMENT OF SPOUSE/CIVIL PARTNER
How to enrol
- Fill in the section regarding family members on the enrolment form if enrolling at the same time as the main member
- Attach the Family Status Certificate indicating the degree of kinship
- If the degree of kinship does not appear in the family status, the “Degree of Kinship Declaration” provided by the Fund can also be attached
- If the spouse is not shown on the Family Status Certificate, a marriage certificate or the spouse’s Family Status Certificate may be submitted
- Attach copy of identity document and health card
- Attach consent to the processing of personal data
ENROLMENT OF COHABITING PARTNER (reserved for working managers)
The enrolment of a cohabiting partner may be applied for from the moment of the manager’s enrolment provided that the cohabitation began at least two consecutive years prior to the date of the enrolment application and takes effect on the first day of the month following the application, while cover begins six months after enrolment.
The enrolment will not be possible if the spouse of an already registered member is present in a family unit already covered by the Fund, without evidence of a decree of divorce.
How to enrol

- Attach the Declaration of De Facto Cohabitation prepared by the Fund
- Attach copy of identity documents and health cards

ENROLMENT OF CHILDREN
Adopted children, or children in temporary pre-adoptive foster care expressly assigned by the competent authorities to a Fasi member, are treated on the same basis as children.
Children up to the age of 18 inclusive
How to enrol:
- Fill in the section regarding family members on the enrolment form if enrolling at the same time as the main member
- Attach the Family Status Certificate indicating the degree of kinship
- If the degree of kinship does not appear in the family status, the “Degree of Kinship Declaration” prepared by the Fund can also be attached
- If no children appear on the Family Status Certificate, birth certificates may be submitted as an alternative
- Attach copy of identity document and health card
- Attach consent to the processing of personal data
- In the case of adopted children or children in temporary pre-adoptive foster care, attach a copy of the adoption decision or foster care order issued by the competent authorities
Children over the age of 18
- Enrolled on a regular pre-university course of study (up to the age of 21)
- Enrolled on a regular university course of study (up to the age of 26)
- Children with a disability of more than 66% (no age limit)
- Income not exceeding the value set annually by the Fund (see Circular for Managers)

- Attach the Family Status Certificate indicating the degree of kinship
- If the degree of kinship does not appear in the family status, the “Degree of Kinship Declaration” prepared by the Fund can also be attached

- Attach certificate of study for children who are students: up to age 21 school attendance certificate or university enrolment certificate, up to age 26 university enrolment certificate only
- As an alternative to submitting a certificate of university studies, it is possible to submit a “Declaration In Lieu of Certification”, pursuant to Article 46 et seq. of Presidential Decree 445/2000 as amended and supplemented, certifying enrolment in the faculty attended, subject to submitting the relevant declaration of liability for tax purposes The relevant declaration in lieu of certification forms, also available from the websites of individual universities attended, must provide full information on::
- year of enrolment;
- legal duration of the course of study;
- academic year attended at the time the declaration was submitted.
- In the case of children with a disability of over 66%, a disability certificate
- Attach a declaration of responsibility signed by the relevant child (or his or her legal guardian) certifying that he or she does not receive income in excess of the limits set annually by the Fund (see Circular for Managers)
- In the case of adopted children or children in temporary pre-adoptive foster care, attach a copy of the adoption decision or foster care order issued by the competent authorities
ENROLMENT OF PARENTS
Parents’ income must not exceed the limits set annually by the Fund (see Circular for Managers)
How to enrol
- Send the application for enrolment of parents simultaneously with the manager’s enrolment, or no later than 60 days after becoming dependent
- Fill in the section regarding family members on the Enrolment Form
- Attach a declaration of responsibility signed by the relevant parent certifying that he or she does not receive income in excess of the limits set annually by the Fund (see Circular for Managers)
- Attach the Family Status Certificate of the dependent parent together with the degree of kinship declaration
- Attach consent to the processing of personal data
